There are at least two ways you can do this:
ADD PERSONAL PHOTOS
This is completely optional of course, but pictures are worth 1000 words.
ANSWER SOME QUESTIONS ABOUT CIRCULATION GUIDELINES
This is important for us to know how much you are willing for us to share your story. Our Family Scrapbook will be shared with others, including legislators and community organizations. Copies of your Personal Story may be made in order to provide our scrapbook to as many people as are interested. In addition, Indy PFLAG will, with your permission below, post your Personal Story on our website. (Anonymity is an option.) Please answer by deleting either YES or NO.
